1. To enroll, call Lauren Kuhn with Progressive Benefit Solutions at 203-208-4841. Please note the following information will be required to get enrolled:
   • Name
   • Date of Birth
   • Social Security Number
   • Physical Address
   • Phone number
   • Email address
    
2. Once you are set up in the system, an email will be sent to the provided email address with a link to complete your enrollment. 
    
3. Please note: Once the confirmation has been signed electronically, you must complete the banking form in the Task section and attach to the enrollment site. This will allow the monthly premium to be deducted from your account around the first of every month.
